Community Home Health & Hospice

Community Home Health & Hospice is made up of 300 employees and 231 volunteers, including nurses, aides, social workers, chaplains, and physicians who serve around 4,300 patients and families in Washington and Oregon. Clients are provided with home health care, hospice, and grief support services. The Park West Foundation donated during the organization’s fundraiser in May 2017 to assist those who cannot otherwise receive hospice care.
